SANAR It is important that the SANAR form be filled out correctly. If incorrect information is entered, it could result in the form being returned or the account being established and you not being able to access it. Accuracy It is essential that the SANAR form be filled out completely. This includes all signatures and dates. If information is missing, the form will be returned for correction, which can slow the account process significantly. Completeness Lorenda M. This form is used to establish new human resources automation accounts for Department of Army personnel or make modifications to existing accounts. This form contains a total of 22 blocks that are meant to capture user information necessary to establish a user’s record as they are nominated for access by their Supervisor or Manager. Once this form is completed, it will contain personally identifiable information and must be encrypted when submitted for processing. Please ensure that you have the latest version of this form before completing it. Block 1. AKO User Id: Insert the Army Knowledge Online (AKO) user name. Ex: If the AKO email address is john.smith@us.army.mil, you would put ‘john.smith’ in this block. Filling out this block is optional. john.smith@us.army.mil Block 7. Date of Birth: This block has a drop-down calendar you can use. If you choose to type the date of birth in manually simply use the MM/DD/YYYY format. Block 3. Name: Insert the name and rank in the following format – LAST, FIRST, MIDDLE, TITLE OR RANK. Ex: If the name is Julie Ann Smith and you are an Army Civilian, you would insert ‘Smith, Julie, Ann and the job title’. Block 5. Action or Access Required: List the system(s) the user will require access to. For new user accounts insert ‘DCPDS/CSU’. You can also insert BOA if you require an account. For account modifications insert ‘Modification to existing account to…..’ Block 2. Submission Date: Place the date that the for is being submitted for processing. Ex: If you are sending the form to the CPAC on July 1, 2014, insert ‘07/01/2014’ here. Block 4. Expiration Date (for access requirement): Leave this block blank Block 6. Social Security Number: Please make sure that the social security number input is correct and complete. If the social security number is wrong, we will not be able to successfully establish the account(s). Block 8. Phone: Please insert both the DSN and Commercial office phone numbers here. Block 14. Name and Title: This block is for the full name and title of the user’s Supervisor or Manager. Block 10. DoD ID/EDIPI: Please insert the 10-Digit number that is located on the back side of the user’s Common Access Card. Block 12. E-Mail Address: Please insert the AKO email address here. Please do not use the enterprise address. Even if you no longer use the AKO, it is still valid and required in order to establish the account(s). Block 9. IP Address (or address range): Please leave this block blank. Block 11. Mailing Address (duty): Insert the office mailing address here. Block 13. Statement of Accountability: Have the user read the statement carefully and then digitally or physically sign in this block. Please ensure that the user has inserted the date in the block to the left of the signature line prior to signing. Block 15. Phone: This block is for the phone number of the user’s Supervisor or Manager. Block 17. Supervisor Approval. This block must be dated and signed by the Supervisor or Manager who is nominating the user for access. Block 19. Phone: This block is for the office phone number of the unit level IA person or the CPAC IMO if the unit does not have an IA contact. Block 16. Organization: This block is for the name of the organization of the user’s Supervisor or Manager Block 18. Name: This block is for the full name of the unit level Information Assurance (IA) contact or the CPAC Information Management Officer (IMO) if the unit does not have an IA contact. Block 20. Email: This block is for the email address of the unit IA person or the CPAC IMO if the unit does not have an IA contact. Block 21. The unit level IA person should verify security authorization for the nominee to perform duties at or exceeding the sensitivity of the required access and then indicate ‘Yes’ or ‘No’ by clicking the appropriate radio button. Organizational IA Contact Approval: This block is for the signature of the unit level IA person or the CPAC IMO if the unit does not have an IA contact. Please ensure that this block is dated.
