2. To work collectively to shape a strategy for people living with and beyond cancer in Northern England SCN & Senate area. To focus particularly on risk stratification of care beyond active treatment and the support required to implement effectively. To leave with a clear commitment to action – as individuals, within localities and through support from the Strategic Clinical Network Welcome & aims of the day

Context 1 in 3 of us will be diagnosed with cancer in our lifetime Fortunately around half of those diagnosed now survive for at least 10 years The number of people living with cancer is predicted to double to around 4 million over the next 20 years Traditional models of follow up are not fit for purpose, not what patients tell us they want and organisationally unsustainable.

National Cancer Survivorship Initiative Legacy Identified four priority areas for implementation/further work: – Recovery package – Risk stratified pathways – Physical activity – Consequences of treatment NHS England Living With & Beyond Cancer Programme NCSI – next steps - adapting to a new environment

5. Five Year Forward View – opportunities and challenges Promotion of well being and independence More engaged relationship with patients, carers and citizens Greater control of own care/management of own health Importance of information for empowerment

6. Five Year forward View – Opportunities and Challenges Don’t be constrained by ‘what is’ – be guided by what is known to work but adapt to local circumstance. Design services to support people with multiple health conditions Integrate across the system, around the patient; System is stretched but through collective action we can deliver improvement.
