What is the African American Health Conductors Program
Is a part of an overall movement to eliminate health inequities and health disparities affecting the African American population. This program was inspired by the life and dedication of Harriet Tubman to the freedom of African Americans.

Why and How did this program begin?
With the first funding from Kaiser Permanente, the African American Health Conductors came on board to help address community residents concerns that the Bay Point Family Health Center only serve the Hispanic population.

Changing the Perception…
Create the feeling of being welcomed. (Provided Cultural Competence and Sensitivity trainings with staff) Surveyed the needs/concerns of 254 African American Bay Point residents. Brown Bag gatherings were held to hear and share each others culture.

Outreach to African American residents in Bay Point/Pittsburg Provide Patient Navigation Conduct Community Health Education Campaigns

Core Functions continued…
Conduct/facilitate Health Support Adult Groups called Soul to Soul and Body & Soul (co-ed). Patient/Client Advocacy

Health Conductors Program Successes for Fiscal Yr. 08-09
Annual Black History Celebration 150 people attended. Rites of Health Ceremony The conductors successfully assisted (navigated) 80 African American men and women thru CCHS Poster campaign, estimated 20,000 people seen poster

Health Conductors Program Success for Fiscal Yr. 08-09
Male Conductor outreach to 550 African American male residents in Bay Point, Pittsburg and Antioch. Thanksgiving Luncheon (focus group) 62 people attended. Kwanzaa 135 people attended. 1st Roll out of African American Nutrition series
