3 Capture, document and reflect

4 Requirements: Completion of forms found on the web (R,S,T,U,V W, X) Written reflection (4 pages) Three pieces of evidence

5 Each committee will maintain the following records: Candidate Score Report (Form V) Candidate’s Written Narrative Committee Requirements

6 Renewal Process Candidates may choose either Year Four or Year Five of their Master Teacher designation Year Four Renewal: Renewal status will take effect when the current five-year designation expires

7 At least two members of the committee will score each application using the Score Report. Committees will provide each candidate with a compilation of his/her final score. Form V Processes and Procedures

8 If the applying teacher is deemed not successful, the teacher shall receive written feedback and an opportunity to reapply if current Master Teacher designation has not expired. Processes and Procedures

9 Each committee will establish an appeal process for teachers Processes and Procedures

10 Unsuccessful Renewal

11 Master Teacher renewal must take place before expiration of a Senior Professional Educator License or Lead Professional Educator License. Unsuccessful Renewal

12 If the Master Teacher renewal is unsuccessful, the candidate will need to apply for a Professional Educator License if applicable Unsuccessful Renewal
