16 EXAMPLE Harry Smith is 6 years-old and came to school today with bruises on both arms. He is quiet and withdrawn. Harry lives with his mother Helen and his little sister Jenny (age 3). His teacher, Susan Ford, is concerned about him and calls her local Children’s Services to investigate.

17 Susan speaks with a member of the duty team who opens CareDirector and records the incoming phone call from Susan. REFERRAL APPROVAL AND ALLOCATION You can configure CareDirector so that this action automatically creates a referral and an initial assessment for Harry Smith.

18 Gerald Walker, the Manager of the duty team, is alerted by email that a new referral is awaiting allocation. He decides to allocate the referral to Jackie Ryan who will work with Harry to assess his needs and plan his support. REFERRAL APPROVAL AND ALLOCATION

HARRY’S NEEDS To ensure assessments are completed within a locally-defined timeframe, CareDirector sets the “Due Date” automatically.

26 Once Jackie’s Manager has authorised the closure of the initial assessment, she creates a core assessment. HARRY’S NEEDS Using the CareDirector workflow engine, the core assessment can be auto-generated after the closure of the initial assessment.

27 Using the CareDirector single data entry facility, Jackie decides to pull forward the information that she previously recorded in the initial assessment. HARRY’S NEEDS

29 In our example, an outcome from the core assessment is to provide Harry with respite care for a week to give the family a break. When the service is recorded in CareDirector, an automated task is created for Jackie’s Manager to authorise the service provision. HARRY’S NEEDS
