2 Putting People First Portsmouth City Council What is the Prevention and Wellbeing Network? A Partnership of statutory services, service providers, voluntary sector organisations and most importantly older people, working together to provide a broad range of preventive services to enable older people to remain living independently in their own homes with an improved quality of life.

3 Putting People First Portsmouth City Council Why have a Prevention and Wellbeing Network? Ageing population providing significant new challenges to health and social care currently and in the future A push by and on behalf of older people and carers for a greater say in the way services are delivered, with an increased emphasis on wellbeing, choice, dignity and respect. A recognition that if demographic, social and health care challenges are to be seriously tackled the delivery of services has to change, to be more inclusive, cost effective and, above all, more effective at meeting the needs of the whole community The Prevention and Wellbeing Network is only one possible model but in Portsmouth we find that it enables us to plan and deliver more joined up holistic services that help older people in the city to remain living independently with a good quality of life.

4 Putting People First Portsmouth City Council So What Are the Benefits of Having a Prevention and Wellbeing Network? Exchange of information enabling services to understand more about each other and older people to understand more about and gain easier access to services. Provide a more whole systems approach by joining up services and creating more single points of access. Avoids duplication of services and ‘silo’ working Identifies gaps in service provision and explores ways of meeting unmet needs. Is a very important interface between older people and service providers Helps raise the profile of older peoples services and ensure that they are part of the new agenda, e.g. LSP, Community Strategy, Local Area Agreement etc. Helps to drive the NSF for older people

5 Putting People First Portsmouth City Council Enables bidding for non-statutory funding streams, e.g. SRB, NRF, AIF,ESF Recognises the real value of community and voluntary sector contribution

6 Putting People First Portsmouth City Council How Does the Network Operate? Agreed to look at 3 levels>primary, secondary and tertiary prevention. BI-annual conferences where older people and service providers look at: Existing provision What has been achieved What needs to change and how this is achieved

7 Putting People First Portsmouth City Council 5 Major themes identified at first conference Health and Social care Homes and Environment Transport Activities Information Action plans produced with measurable targets. Driven and monitored by BI-monthly Prevention and Wellbeing Network meetings and Bi-annual conference. Supported by the DSS Prevention Team NB. The needs OF older people Do Not remain static and service provision needs to change accordingly.

Recognition of the value of the Network and possibilities of achieving increased external funding Older persons elected member champion Older peoples ‘Champions’ in different department / organisations A significant increase in knowledge of what is going on in terms of older peoples services The opportunity to be innovative and think ‘out of the box’ Has made the involvement of older people in the development of services more real.

10 Putting People First Portsmouth City Council Some Examples Fuel Poverty and Winter Warmth: Substantial progress with joint work between PCT, DSS, Housing, Environmental Health and Pensions Service. Dvd ‘Eat or Heat’ made by older people, for older people if to get message across Falls Prevention Huge progress. Funding for jointly appointed City Falls Prevention post ( PCT/DSS ); engagement of a broad range of partners in city and district LIT groups; jointly funded prevention programme. Significant improvement re performance indicator - “ steady improvement”. Dvd ‘I Fall’ made by older people for older people to get the message across.

11 Putting People First Portsmouth City Council Targeted information for older people O.P information and taster days to provide older people with access to information about services and opportunities for tasters re leisure and learning opportunities Eat for Health web-site Companion guide to services in inner city areas. Home Safety Reminder Work with older persons specific radio station, including training older people to become programme presenters etc. Funding achieved for post to encourage retired older people from inner city to access a wide range of leisure and learning opportunities. Funding achieved for mid-life intervention post Funding achieved for posts to provide exercise ‘Health through Stealth’ for older people across the City Funding achieved for post to provide gardening opportunities in older persons residential establishments

12 Putting People First Portsmouth City Council Intergenerational Scheme Has involved elders going to help children in the inner city schools and children assisting the elders with I.T projects. Drama project with elders working with excluded young people from inner city schools Targeted work, with elders from the Bangladeshi community working with children from inner city junior schools.

13 Putting People First Portsmouth City Council Carers Services Appointment of dedicated older persons carers development worker to ensure that the needs of older people and their carers are clearly identified and supported

14 Putting People First Portsmouth City Council Home Safety Campaign Fire service completing home safety checks for older people as well as free supply and fitting of smoke detectors. Development of a home safety reminder list to be used by all service providers visiting older peoples home to ensure that hazards are indentified and correctly reported. Joining up of handy person and care and repair schemes to provide a single point of access.

15 Putting People First Portsmouth City Council Contracted Services All providers now part of Prevention and Wellbeing Network and more joint involvement in planning and delivery of contracted services rather than previous climate of non-co-operation and competition.

17 Putting People First Portsmouth City Council Challenges Older persons services continue to be the ‘poor relation’ when compared to some other services. Prevention is still not fully understood and continues to be the first to be cut in times of budgetary constraints. Innovative thinking in terms of prevention is often not seen as the business of DSS/PCT Still difficult to get some services to ‘The Table’. Funding is never sufficient to meet all the needs.

18 Putting People First Portsmouth City Council The next steps Major conference in September this year to review progress, celebrate success and make decisions about future service developments Ensure that older peoples issues are core to the LSP and LAA agendas Develop new performance indicators that encourage and enable a wellbeing approach and lead to increased local flexibility and ownership Ensure that performance indicators are a part of an overall plan and not separate measures of isolated activity by developing an outcomes based approach.

19 Putting People First Portsmouth City Council Outcomes: Identify clear positive qualitative statements of wellbeing which are interactive and independent. “Elders live with dignity and independence in settings they prefer” Indicators: Develop qualitative indicators over time which are interactive, help to compare with others and motivate to improve. “Are we getting better or worse” Strategy: Agree a common purpose and a set of coherent collective actions that will deliver improved outcomes for older people. “What can your organisation do to improve the wellbeing of the older people it serves?” Above all ensure that older people and carers are fully engaged in the planning and delivery of the new services. We must stop seeing them as the problem and start seeing them as part of the solution.
