2 What Is This ‘Legal Packet’?
In this packet you should have received the following: Opening Letter (Single Page) Enrollment Instructions and Information (Single Page) Joinder Agreement (Legal Document) Master Trust Agreement (Legal Document) The Arc of Washington State

3 Joinder Agreement… What’s That?
This is the legally binding document that you need to eventually fill out and have signed and notarized It contains info about the Trust you need to know (and) It contains spaces for you to fill in your own info The Arc of Washington State

4 What Info Do You Need To Fill In?
Let’s start at the beginning (Page 3) of the Joinder Start (p.3) Finish You are here Please note that pages 1 – 2 provide definitions of terms used in this agreement and may be a helpful reference. The Arc of Washington State

5 (p.3) Primary Donor & Beneficiary
This is the person who is setting up, filling out the documents, and submitting the trust fund application. This is the person with the developmental disability for whom the trust is being established. The Arc of Washington State

6 (p.4) Legal Representative & Primary Representative
This is the person who is a legal representative for the beneficiary. (This could be a guardian or rep. payee. Also, if the beneficiary is under 18 the parents are the legal guardians) This is the person who will request disbursements and receive the quarterly statements for the account. The Arc of Washington State

7 (p.5) Primary Representative Alternates
For this page please list who you would like to be primary representative if the first person listed is no longer able to serve. The Arc of Washington State

8 (p.6) Current Government Benefits
Please list any forms of local, state, and government assistance that the beneficiary receives, including Medicaid and SSI. Benefits The Arc of Washington State

9 (p.7) Insurance Policies and Disability
Health and Burial Policies Disability Describe the beneficiary’s developmental disability. Is the beneficiary covered under any health insurance policy, or prepaid burial insurance plan? The Arc of Washington State

10 (p.8) Disability (Continued)
Greatest Challenges In as much detail as you like, describe what you think will be the beneficiary’s greatest challenges in the future. The Arc of Washington State

11 (p.9) Disposition Plan In this section you will list where the money goes, if the beneficiary dies, and there is a balance remaining in their account. If you want to leave money to other individuals involved with this program place a % here If you would like to leave money to other persons or organizations place a % here The Arc of Washington State

12 (p.10) Disposition Plan If you have additional persons or organizations please list them here Please note that the totals from all boxes need to equal 100% The Arc of Washington State

There are no sections to fill-in on these pages. Even though the legal language may seem a bit overwhelming it really is straightforward and we encourage you to read these pages. Are we finished yet? The Arc of Washington State

14 (p.14) Signature Page On the bottom of page 14 you will find:
A space to fill in the date A line title ‘Primary Donor’ for you to sign. Do not worry about the lines below this, those are what we sign when we receive your Joinder. Do not sign your Joinder until you are with the notary. They notarize that it is your signature. The Arc of Washington State

17 Schedule C Disbursement Plan
For this schedule the primary donor is asked to identify goods and services the beneficiary may need for future disbursements. You can go into as much detail as you like. We realize these may change as years pass. Just because you do not check something now, does not mean you cannot request a disbursement for it later. 18 What Can You Amend? Can Amend Can’t Amend Legal Representative

19 What To Do After The Joinder Is Complete?
To complete enrollment you will need two more things: $600 enrollment fee check Letter of Eligibility The Arc of Washington State
