Overview of Reporting Requirements Quarterly Reports Due 30 days after each quarter: (Oct 30 & Jan, April, July) 1.Action Plan with “Steps Completed” 2.Quarterly Feedback Report **nice work in Q1 on both of these! 3.Cumulative Fiscal Report (on eGrants) **If you’re not fairly close to 50%, lets talk in Feb. Add’l Mid Year and Year End (Jan 30, July 30) 4.Progress Report Add’l Year End (July 30) 5.Long Term Objectives Year-End Status **We’ll talk about this more in Q4

Mid year progress report Standardized form to be completed via Survey Monkey Will request information on: – Schools/students involved in grant activities – District staff involved in the grant – Staff training & professional development – School Wellness team composition – Involvement in Alaska Alliance for Healthy Kids/other local coalition** – Completion of Year 1 milestones and/or Barriers – Wellness Policy Implementation & Enforcement – District priorities re: Student Health Measures

Mid year progress report Standardized form to be completed via Survey Monkey Expect email link before Dec. 30 o To complete for grant activities July 1 st through Dec 30 th Don’t feel pressured to “say yes” to everything o You are NOT expected to be done with all of these items o You are roughly 3-6 months into a 4 year project o Opportunity to give feedback on assistance needed Please also give feedback on the format if needed o Questions are difficult to answer, something is missing, etc. o We will provide a pdf copy to review if you have internet connectivity issues
