3. Log in with your med Username and Password

4. It’s the Same Log in that You’ve been Using for CDCS

5. Select the program that you need. You should only choose M3 & M4 Clerkships ESSENTIAL STEP!!!

6. Your next screen will identify you and all of the roles you have in that program.

7. If you have evaluations pending, simply clicking on the link will take you to the pages where it can be completed, submitted or saved for later.

8. When you click on the Pending Evaluations, you will see something like the screen below Click on Edit Evaluation

9. The Evaluation will open, and you will see the pertinent details in the header. You will also see a picture of the student

10. Note that you may Save your work for Later and that if you choose to Submit, the form will no longer be visible.
