State Primary Care Grants Program Eligibility: Public and Non-Profit entities Public and Non-Profit entities Provision of Primary Care services Must serve medically underserved population (low income, uninsured, disadvantaged minorities) Cannot replace “government” insurance (Mcaid, Mcare, etc.)
