3 LEARNING OBJECTIVES Identify purpose of bathing. Describe equipment that used in bathing. Perform bathing procedure.

4 Bathing Maintaining personal hygiene is essential for the preservation of health and the prevention of infection, and removal of body odor.

5 Purpose To remove transient microorganism, body secretion and excretions and dead skin cells. To stimulate circulation to the skin.

6 Purpose To produce a sense of well-being. To promote relaxation and comfort. To prevent or eliminate unpleasant body odors.

7 Assessment Condition of the skin (color and presence of pigmented spots, temperature, lesion. Fatigue. Presence of pain and need for adjunctive measure (e.g. an analgesic) before the bath.

8 Assessment Range of motion of the joints. Any other aspect that affect the client bathing process (e.g. mobility, cognition, strength) Need for use of clean gloves during the bath.

9 Equipment Basin or sink. Warm water Soap. Soap dish. Bath blanket. Two bath towel. Washcloth. Clean gown or pajamas. Gloves if needed. Personal hygiene articles (optional). Table for bathing equipment. Laundry hamper.

10 Determine: The purpose of bath. Self-care ability of the client. Any movement or positioning precaution specific to the client. Other care the client may be receiving, such as physical therapy or x-ray, in order.

11 RationalAction Foster the client’s cooperation and understanding. Reduce transmission of infection. 1- Explain to the client what you are going to do. 2-Wash hand..

12 RationalAction Reduce transmission if infection when touch the body fluid 3- Wear gloves if needed. 4-Provide client privacy and quit environment.

To avoid straining and promotes good body mechanics. 5-Prepare the bed and positioning the client appropriately -Position the bed at comfortable working height. - Lower the side rail on the side close to you. - Keep the other side rail up. - Assist the client to move near you.

14 RationalAction The bath blanket provides comfort, warmth and privacy. 6-Place bath blanket over the top sheet 7-Remove the top sheet from under the bath blanket starting from shoulder and moving down toward feet.

15 RationalAction 8-Remove the gown while keeping the client covered with bath blanket. 9-Place gown in linen hamper..

16 RationalAction Begin the bath at the cleanest area and work downward toward the feet 10-Make a bath mitt with washcloth, using triangular method or rectangular method. 11-Wash the face.

17 RationalAction To protect the bed from becoming wet. To prevent transmitting microorganism from one eye to other 12-Place towel under head. 13-Wash the client’s eye with water only and dry them well, use a separate corner of the washcloth the each eye.

18 RationalAction To prevent secretion from entering the noselacrimal ducts. 14-Wipe the eye from the inner to outer. 15-Ask the client if wants soap used on the face. 16-Wash and rinse the face.

19 RationalAction To protect the bed from becoming wet. 17-Dry the client face, ear and neck. 18-Remove the towel from under the head. 19-Place the towel lengthwise under the arm nearest you..

20 RationalAction To promote circulation by increasing venous blood return 20- Wash and rinse the arm and the hand, starting from the distal to proximal area including the auxiliary area..

21 RationalAction 21- Dry the arm by elevating the client’s arm and supporting the client’s wrist and elbow. 22- Apply deodorant or powder if desired.

22 RationalAction Many client enjoy to immersing their hand and washing themselves. Soaking loosens dirt under the nail. 23-(Optional) Place a towel on the bed and put a washbasin on it. Place the client’s hands in the basin..

23 RationalAction Keeps the client warm while preventing unnecessary exposure of the chest. 24-Dry the hand particular to the spaces between the fingers. 25- Place bath towel lengthwise over chest.

24 RationalAction 26- Fold the bath blanket down to the client’s pubic area. 27- Lift the bath towel off the chest. 28- Bathe the chest and abdomen.

25 RationalAction 29- Give attention to the skin under the breasts and any other skin folds. 30- Rinse and dry well. 31- Replace the bath blanket when the areas have been dried.

26 RationalAction To promote privacy and maintains the client dignity 32- Expose the leg nearest from you by folding the bath blanket toward the other leg being careful to keep the perineum covered. 33- Lift leg and place the bath towel lengthwise under the leg.

27 RationalAction To promote the circulation by stimulating venous blood flow. 34- Wash, rinse and dry the leg from the ankle to the knee to the thigh. 35- Wash the feet by placing them in the basin of water. 36- Dry the foot, particularly the spaces between the toes

31 RationalAction 45- Place the towel under the leg. 46- Wash, rinse and dry the leg. 47- Wash the feet by placing it in the basin of water..

32 RationalAction 48- Dry the feet, particular between space of toes 49- Assist the client into prone or side-lying position facing a way from you.

33 RationalAction 50- Place the towel lengthwise alongside the back and buttocks. 51- Keeping the client covered with the bath blanket.

34 RationalAction 52- Wash and dry the client’s back, moving from shoulders to the buttocks, take attention to gluteal folds. 53- Perform a back massage.

35 RationalAction 54- Assist the client to the supine position. 55- Help the client put on a clean gown or pajamas. 56-Rise the side rail.

36 RationalAction 57- Remove the bath blanket, and put it in the hamper. 58- Cover the client with clean top sheet.

37 RationalAction Foster the client’s cooperation and understanding. Reduce transmission of infection. Reduce transmission if infection when touch the body fluid. 59- Document the type of bath given: 1- skin assessment, 2- nursing intervention related to skin integrity, 3- ability of the client to assist or cooperate with bathing 4- educational needs regarding hygiene.
