3 Grade “A” Laboratory Evaluation Request and Agreement

5 Lab Evaluation Request and Agreement Form Signed by Lab Rep and Lab Evaluation Officer prior to laboratory evaluation Copy can be left with laboratory Original sent to Albany

6 Daily Refrigerator Temperature Record

8 Refrigerator Temperature Record Temperature must be recorded twice daily AM & PM records required Ex. Once from each shift. All dates should be filled in. If lab is closed, specify that the lab is closed. Temps out of range must be corrected and re-recorded

9 Daily Freezer Temperature Record

11 Freezer Temperature Record Temperature must be recorded twice daily AM & PM records required Ex. Once from each shift. All dates should be filled in. If lab is closed, specify that the lab is closed. Temps out of range must be corrected and re-recorded

14 Provided form is a sample that can be modified to be approved by the assigned laboratory evaluation officer.

15 Required Information For Drug Test Load ID Tester’s Initials Pounds of Milk Time of Test Temp of sample at test time Numerical Test Result Interpretation of Numerical Test Result

16 Controls Positive Control Tested prior to any truck testing Obtain appropriate result as prescribed by the manufacturer If found “not positive”, retest with another kit first. ID with date/time, lab expiration Not-Found Control Test prior to any truck testing, must be from known “not-found” supply (this supply must be identified) Obtain appropriate result as prescribed by the manufacturer Only acceptable result is “Not-Found” Retest with another kit if any other result obtained

17 Calibrators Used to ensure proper functioning of the readers Each reader provides results within a specified range Results of the calibrators must be recorded Testing is to be discontinued and the manufacturer contacted if the specified range is not met.

18 Test Kit Drug Suitability Check

20 Drug Test Kit Suitability A report form such as the one provided must be utilized to record results of Drug Test Kit Suitability Upon receipt of a shipment of new test kits a positive and “not-found” control must be run Appropriate results must be found Test kits cannot be used if appropriate results are not obtained.
