2 2 SUPERVISOR, ATTENDANT and ENTRANT zMUST KNOW: yHazards that may be faced during entry yMode, signs or symptoms, and consequences of exposure

3 3 DUTIES OF ENTRY SUPERVISOR zRESPONSIBLE FOR DETERMINING IF ACCEPTABLE ENTRY CONDITIONS ARE PRESENT zVerify that the appropriate responses have been made on the permit zVerify that all tests specified by the permit have been conducted zVerify that procedures and equipment specified by the permit are in place

4 4 ENTRY SUPERVISOR cont. zACCEPTABLE ENTRY CONDITIONS cont. zVerify that rescue service, if required, is available and that the means for summoning them are operable zSign the permit indicating that entry may begin

5 5 ENTRY SUPERVISOR cont. zACCEPTABLE ENTRY CONDITIONS cont. zEnsure removal of unauthorized individuals who enter or who attempt to enter the permit space during entry operations

6 6 ENTRY SUPERVISOR cont. zAUTHORIZE & OVERSEE ENTRY zDetermine that entry operations remain consistent with terms of the entry permit and that acceptable entry conditions are maintained at intervals dictated by the hazards and operations performed within the space, and whenever responsibility for entry operations is transferred

7 7 ENTRY SUPERVISOR cont. zTERMINATES ENTRY zWhen operations covered by permit are completed zWhen a condition that is not allowed under the permit arises in or near the space

8 8 DUTIES OF ATTENDANTS zAN INDIVIDUAL STATIONED OUTSIDE ONE OR MORE PERMIT SPACES WHO MONITORS THE AUTHORIZED ENTRANTS AND PERFORMS ALL ATTENDANT’S DUTIES ASSIGNED IN THE PERMIT SPACE PROGRAM zMust know hazards that may be faced

9 9 ATTENDANT cont. zRemain outside the space unless relieved by another attendant zKnow possible behavioral effects of hazards in entrants

10 10 ATTENDANT cont. zContinuously maintain an accurate count of authorized entrants in the permit space and ensure that there is a means for identifying authorized entrants (ie. Rosters, tags)

11 11 ATTENDANT cont. zCommunicate with entrants to monitor their status zAlert entrants to evacuate if attendant detects: yprohibited condition ybehavioral effects of hazards in entrants ydangerous situation outside space yThat s/he can no longer perform as attendant

12 12 ATTENDANT cont. zSummon rescue/emergency services as soon as it’s determined entrants may need assistance to escape

13 13 ATTENDANT cont. zUNAUTHORIZED PERSONNEL zWarn them that they must stay away zIf they have entered the space, advise them to leave immediately zInform authorized entrants and supervisor if unauthorized personnel have entered the space

14 14 AUTHORIZED ENTRANT zAN EMPLOYEE WHO IS AUTHORIZED BY THE EMPLOYER TO ENTER A PERMIT SPACE zMust know hazards that may be faced during entry, including signs, symptoms, and consequences of exposure zProperly use equipment (ie. Testing and monitoring, ventilating communication, rescue, etc.) d(4)

15 15 AUTHORIZED ENTRANT cont. zMust communicate with the attendant as necessary to enable the attendant to monitor their status and to enable the attendant to alert entrants of the need to evacuate the space

16 16 AUTHORIZED ENTRANT cont. zENTRANT MUST ALERT ATTENDANT WHENEVER: zEntrant recognizes any warning sign or symptom of exposure to a dangerous situation zEntrant detects a prohibited condition

17 17 AUTHORIZED ENTRANT cont. zENTRANT MUST EVACUATE AS QUICKLY AS PRACTICAL WHENEVER: zAttendant or supervisor orders so zEntrant recognizes warning signs/symptoms of hazardous exposure zEntrant detects prohibited condition zEvacuation alarm is activated
