2 LTI no 29: Finger crushed 25/June/2000 When the Driller had ascertained that all 16 nuts had been threaded on to the stud bolts he informed the workmen to stand clear and he signalled for the Annular BOP to be lowered.

3 What caused this LTI: 1) Injured party did not follow the procedures. 2) Injured party not concentrating on the task being carried out. Discuss in your team: - Do you conduct tool-box talks for every operation ? - How would you have removed or tighten the bolt ? - How can you “see” the hazard/danger of a task/job ?
