*Disposition of Students Did the student return to class? Go home? Parent request? Nurse? Was the student transported by EMS? Was the student sent to hospital, SBHC or health care provider’s office by other means? Other / Specify such as police or CYFD 26

Remember: The total number of student visits = the total number in the disposition section. Only count the student in one category of each visit and list disposition each time. 27
