Benefits of the Approach Prevents / Delays people needing social care and acute services Independence and reablement focus Builds community resilience Identifying the most vulnerable means support can be ‘dropped-in’ at times of greatest need (e.g. winter) rather than create a dependency

He was referred by his GP due to being isolated, having a cold home and a series of falls. He also has Cerebral Palsy. “I am very happy with what you have put in place for me and I am very grateful to my Doctor for referring me. I could have been in 'real trouble' when I fell in the shower as it was very cold in the bathroom. I used my care alarm and was impressed with the way in which the they were able to get me off the floor using an inflatable air bag. I can’t thank you enough, I am warmer, safer and happier since you came along.”

The GP made the referral due to benefit changes that were affecting Mr A.
