5. Hwy Ops Div5 REPORTING  FHWA-810: Bid tabulation  Is required for all F-A const. project on the NHS.  Submit to Division Administrator.  Certified by STA official.  Show details on 3 low bidders.  Submit within two weeks of the award of contract.

Hwy Ops Div6 REPORTING  FHWA-47: Record of materials  Is required for all projects on the NHS that are $1 million or more.  Contractor completes at end of contract and submit to STA.  Information on final costs of wages, salaries and materials used by the contractor.  State DOT personnel verifies information.

Hwy Ops Div13 USEFUL HINTS  Always report your project in a timely manner.  Make sure all information is completed on the forms.  Include your name and the State in the subject line of the e-mail.  Not reporting the data may cause your State to lose funding.  FHWA forms: www.fhwa.dot.gov/programadmin/contracts/  The States are to use the new FMIS Improvement type codes, not the work type codes.
