2. What is SBAR? A method of succintly passing critical medical information between clinicians. Poor communication and lack of situational awareness play a part in many adverse patient events It improves patient safety.

3. When to use SBAR When emailing clinicians to ask for advice or for prescribing requests. When talking over the phone or through VTC. It is a framework – it can be adapted and might not be suitable for every case.

6. Background Pertinent background information related to the situation could include the following: – Medications and allergies – Pathology results – Recent examination findings – Past medical history

7. Assessment This is what I think the problem is: The problem seems to be cardiac infection neurologic respiratory etc. I am not sure what the problem is but the patient is deteriorating. The patient seems to be unwell and may get worse, we need to do refer.
