7 Expansion of Host Homes  Host Home Legislation: Previous Georgia law and regulations limited host family homes to one person needing residential services. HB 970 was approved in FY ‘08 to allow two people with DD to live in Host Home/Life Sharing situations.  Number of persons in host homes:  FY’04 – 160  FY’08 – 408  FY’09 - 800

11 Community Care Service Provider Program  CCSP consumers in CCSP live an average of over 4 additional years in the community. This represents a 44% increased consumer length of stay in the community since SFY 2000  With the Money Follows the Person grant in 2008 and 2009, 56 clients were admitted to CCSP from nursing homes.

12 Other efforts: Aging-Disability Resources  Aging-Disability Resource Centers are single points of entry to access information and LTC services for individuals with disabilities of all ages.  ADRCs keep individuals in the community and out of institutions.  ADRCs currently serve 75% of Georgia's population and are accessible in 118 of Georgia's 159 counties. In SFY ‘09 they assisted 106,279 Georgians.
