3 Privacy Officer Scott Woolley will be designated as the initial Privacy Officer All concerns and questions about our privacy practices need to be forward to Scott

4 Notice of Privacy Practices “Notice of Privacy Practices” Notice MUST be given at admit, including subsequent admissions Receipt to be documented on the NEW Admission Agreement All current Patients need to receive “Notice” Review Notice

6 Admission Agreement The new Admission Agreement will need to be used for all new admissions starting NOW

7 Privacy Policy Horizon Home Health will protect the Privacy and Confidentiality of Personally Identifiable Patient Information Employees will be required to maintain the privacy of the information they have about patients Termination of Employment may occur if they release information without the correct authorization

8 Privacy Policies Patient Authorization is required except for the following: –The Individual –Treatment, Payment, Healthcare Operations –Requirements of Law

9 Privacy Practices General Principle – “Minimum necessary” Personnel who do not have need to know should not have access to information Secure Location in each Office Marketing uses restricted

10 Employee Training New Material for training to be coming soon Review new policies / forms with employees at next inservice Web-based training coming

11 Securing Records We need to create a secure location in each office where non-employees can wait to meet with employees You must “Secure” your area before any unauthorized individual enters you area Records need to be locked – same as before
