FORM 1000S – APPLICATION TO CONDUCT SEISMIC OPERATIONS This presentation will assist you in the completion of the Form 1000S. The application to conduct seismic operations. Date of Last Revision: 2008. Rule Requirement: OAC 165:10-7-31(b) Explanation of Form: A permit for seismic operations must be obtained. The applicant must post a $50,000 bond with the Surety Department in the Oil and Gas Conservation Division. The application must also be accompanied with a pre-plat of the project area. [Reference 165:10-7- 31] The bond posted for seismic operations is a performance bond that used on a seismic shoot to shoot and is totally separate from any other type of surety. FORM 1000S – APPLICATION TO CONDUCT SEISMIC OPERATIONS COUNTY OR COUNTIES : this is the county or counties of where the actual seismic shoot is going to occur

FORM 1000S – APPLICATION TO CONDUCT SEISMIC OPERATIONS TYPE OF SEISMIC SHOOT: mark one or both whichever applies. All the information in this section should be consistent with the type of operations that the operator is applying for.

FORM 1000S – APPLICATION TO CONDUCT SEISMIC OPERATIONS PLAT OF SHOOT AREA: make sure that the plat is legible and has section, township and range clearly marked! If the OCC cannot read the plat it will slow down the application process.

FORM 1000S – APPLICATION TO CONDUCT SEISMIC OPERATIONS APPROXIMATE DEPTH OF HOLES TO BE DRILLED: check the number of holes and the size of the charges to make sure these are correct for your current operation. Frequently, these are left over from another shoot and not updated each time.

FORM 1000S – APPLICATION TO CONDUCT SEISMIC OPERATIONS FINANCIAL SECURITY GURANTEE: Just remember is totally different from any other type of surety that the operator may have on file with the OCC

FORM 1000S – APPLICATION TO CONDUCT SEISMIC OPERATIONS ESTIMATED DATE OPERATIONS TO BEGIN: Please make sure your date is reasonable! Plan ahead and give yourself some lead time. If there are questions about your permit or if your application arrives on a Friday and you want to start over the weekend that may not be possible.

FORM 1000S – APPLICATION TO CONDUCT SEISMIC OPERATIONS DISTRICT OFFICE TO NOTIFY: this can save lots of headaches as far as dealing with land owners. If everyone knows what is going on. Get to know your field inspector for the area, he can really help you out.
