2 After two (2) years of profiling and assessing NHTS/CCTs unmet needs …..... have we provided services to those we found with unmet need?

3 Are Identified Probable Users Getting FP Services? Unserved probable users in current month are added to succeeding month. Acceptors are also cumulative Service provision not keeping up with identified probable users

4 Reporting Protocol CHT Partner uses the Logbook in profiling, master listing, giving of messages and accomplishing of Family Health Record Form NDP checks the entries in the LOGBOOK and helps the CHT Partner in: Correctly filling-out the logbook, Family Health Record Form and Masterlist Polishing the messages RHM provides overall supervision to CHT Partners and NDP, provides services including counseling, records in the facility and home-based records, provides appropriate health messages and uses the logbook to find out who else needs services and should be followed-up by the CHT Partner
