Showing how Better Futures can be used flexibly to develop a person centred support plan including how the system can be tailored for individuals where maintenance rather than “progress” is the goal.

Aspire as an organisation We believe in the general principle that people are greater than they think they are. Our Vision To be an organisation that can consistently evidence that it keeps its promise: in doing everything possible to put the interests of the individuals with whom we work first and foremost in all that we do; an organisation that can be trusted to continually deliver highly flexible effective and cost-efficient services that demonstrably make a positive difference to those persons’ lives and benefit their families and local communities.

Person-Centred Support Our society is based on the belief that everyone has a contribution to make and has a right to control their own lives. Services should be person-centred, seamless and pro- active. They should treat people with respect and dignity, supporting them to overcome barriers to inclusion. They should support independence, not dependence. They should focus on positive outcomes and well-being. (Department of Health, 2005)

Better Futures within Aspire Resettlement services – 6 to 9 months stay, baseline assessment completed within first week of individual residing in the service. Staff are facilitator of the tool, supported individual is in control of all aspects of their support and how this is provided. Learning through shared action. Development plans derived from scoring and discussions held during Better Futures review. Holistic assessment of needs which focuses on positive outcomes and how this can be achieved using creative solutions.

Better Futures within Aspire People compile their own development plans if they are able to. If not, staff will scribe for them ensuring that they are still fully in control. This enables people to see for themselves their potential and has a vast impact on peoples confidence and self-worth. Development plans are only the beginning of the process, the development plan is not the outcome and is a description of how the outcome will be achieved. Development plans change as needs of person changes. Reviews are paramount to an individuals journey and should occur at regular intervals (4-6 weekly) showing how an individual has progressed or remained stable over this period of time meeting positive outcomes either way.

I am managing my addiction and have dramatically reduced my daily prescribed methadone from 95mls to 25mls in the space of 4 months I am attending counselling due to recent bereavement and am developing coping mechanisms which are helping me I am attending Phoenix Futures as part of my recovery I am managing tenancy with no support from staff and am looking forward to accessing my own tenancy

I’m attending Coach House Trust 3 days per week and Restart one day per week. Myself and the staff have observed significant improvement in relation to my mental health and development of coping mechanisms. I’m managing my own tenancy successfully. My move-on plan is making significant progress due to these positive outcomes being achieved.
