Seek an agency that will provide you with a good learning experience and will allow you to advance your career. http://ctr.wp.uncg.edu/current-students/practicum-internship/

Practicum and Internship Forms Required Before Beginning Hours The purpose of this form is to indicate your intent to enter into a practicum or internship. This form must be completed prior to the start of your fieldwork experience. Form #1 Eligibility Form for Practicum or Internship http://ctr.wp.uncg.edu/current-students/practicum-internship/

Practicum and Internship Forms Required Before Beginning Hours This form is used to obtain approval for the practicum or internship site experience your advisor has approved. This form must be completed prior to the start of your fieldwork experience. Form #2 Request for Practicum or Internship Site Approval http://ctr.wp.uncg.edu/current-students/practicum-internship/

Practicum and Internship Forms Required Before Beginning Hours This form must be submitted along with a check for the amount shown on the form. If an agency provides liability insurance for it’s interns, a letter must be submitted (on the agency’s letterhead) stating you will be covered for liability. Note: Students completing practicum or internship at a clinical facility must use the “Clinical” form, students at non- clinical sites must use the “Non-Clinical” form. This form must be completed prior to the start of your fieldwork experience. Form #3 Liability Insurance Form http://ctr.wp.uncg.edu/current-students/practicum-internship/

Practicum and Internship Forms Required Before Beginning Hours This form is completed in conjunction with your University Supervisor, and specifies the dates that required forms and reports are due. This form must be completed prior to the start of your fieldwork experience. Form #4 Practicum/Internship Report Calendar http://ctr.wp.uncg.edu/current-students/practicum-internship/

Practicum and Internship Forms Required Before Beginning Hours Students completing internships at a clinical site must obtain a signed Clinical Instructional Agreement. This form is not needed for practicum students.Clinical Instructional Agreement Using the Agencies Agreement If the agency has its own Clinical Instructional Agreement (sometimes called an "Affiliation Agreement"), the student should request that the agency scan the agreement AS SOON AS POSSIBLE to Amy Chandler at ctr@uncg.edu. Because the agency's agreement must be approved by the UNCG Attorney, it can take several weeks for the agreement to be finalized. Students should seek information regarding the agreement early, so there is enough time for processing before the internship begins. Using the UNCG Agreement If a clinical agency does not have their own agreement, the UNCG Clinical Instructional Agreement can be used.UNCG Clinical Instructional Agreement 1. The student sends the agency the link to the UNCG Clinical Instructional Agreement with instructions to sign where required and scan to Amy Chandler at ctr@uncg.eduUNCG Clinical Instructional Agreement 2. Once the agency signs and scans the agreement to Amy Chandler, the agreement is complete. If the agency has issues with parts of the agreement, they should make notations on the agreement and scan a copy to Amy Chandler at ctr@uncg.edu. The agreement will then be routed to the University Attorney who may or may not sign with his/her approval for the agencies notations on the agreement. At times (but very rarely) the agency and university cannot come to terms with the agreement, and the student has to find another internship site. For this reason, it is very important that the Instructional Agreement be dealt with early, so there will be plenty of time to find another internship if an agreement cannot be completed. 17 Practicum and Internship Agency Evaluation Form Agency Evaluation of Student Intern This form is used for the agency to evaluate the student. Please note: There is a specific form for practicum mid-term, practicum final, internship mid-term, and internship final. Be sure download the correct form. http://ctr.wp.uncg.edu/current-students/practicum-internship/

Practicum and Internship Student Evaluation Form This form is completed by the student, and allows him/her to evaluate the agency and the practicum or internship experience. Student Assessment of Internship Experience http://ctr.wp.uncg.edu/current-students/practicum-internship/

Practicum and Internship Fee Reimbursement A student completing a practicum or internship that is over 50 miles from the UNCG campus is eligible for a refund of the Health and Activity fees. If you are eligible for a refund, submit this form prior to the start of your fieldwork experience. http://ctr.wp.uncg.edu/current-students/practicum-internship/

Financial Aid A student who receives financial aid in the summer, must complete all practicum or internship hours by the time summer grades are due. Otherwise, a grade will not be recorded and the following semester’s financial aid will be negatively impacted. Example John receives financial aid for his summer 2013 internship. John begins his internship in late May 2013, so he will not complete all of the required hours by the time summer 2013 grades are due to the Registrar’s Office. John does not have a grade turned in for summer 2013. John’s financial aid for next semester class (fall 2013) is negatively affected because he did not complete all hours and receive a grade for his summer internship. http://ctr.wp.uncg.edu/current-students/practicum-internship/

Practicum and Internship Important Notes REMEMBER!!!  You must register and pay on time for a practicum or internship!  Start looking for an agency early in case your plans fall through.
