LIC-33LIC-33 Business Address Verification LIC-33 The office and/or shop location for the following licensees must comply with all requirements established in the NYC Zoning Resolution: Plumber, Fire Suppression, Rigger, Sign Hanger, Oil Burner and Elevator agencies. These licensees shall operate a business location in the city of New York at the time of issuance and during the maintenance of their license. Licensees shall submit to the commissioner any changes to their business location within 30 days of such change for verification.

Oil Burner Installer Bond The Code Book Requires Oil Burner Licensees to submit in lieu of our Liability Insurance.

Please note that ACORD forms are NOT acceptable proof of New York State worker’s compensation or disability benefits insurance coverage. Revised Form C-105.2 is effective December 1, 2003. Earlier-dated versions of the form are obsolete and should no longer be accepted by government agencies after that date. Revised Form D-120.1 is effective September 1, 2006. To comply with Worker’s Compensation Law, business must: A- Be legally exempt from obtaining worker’s comp/ disability benefits insurance coverage; or B- obtain such coverage from insurance carriers; or C- be self-insured

Requirements for a Certificate of Worker’s Compensation Certificate Holder is Department of Buildings License number should appear on the certificate Insurance Company must be listed Insured business name must appear exactly as it does on the company’s filing receipt Your business address and phone number must appear exactly as in our system (BIS) C105.2 (12-03) Policy number and effective period must appear

Cancellation Cancellation Of Worker’s Compensation This certificate is sent to Department of Buildings directly from NYS Insurance Fund. In order to have your Certificate of Worker’s Compensation reinstated, you must provide the Department of Buildings with an updated certificate dated after the cancellation notice.

WC/DB 100 (7/04) Affidavit for New York Entities with No Employees, that NYS Worker’s Compensation and/or Disability Benefits is not Required. This Affidavit must be notarized and stamped by NYS Worker’s Compensation Board, therefore original form must be mailed or dropped off to our office in order to be updated.
