1 What is 100,000 Homes campaign? National movement House vulnerable and chronically homeless individuals and families 100,000 by July of 2014.

2 What is Vulnerability Index (VI)? Began as research into causes of death among chronically homeless individuals Tool for indentifying and prioritizing people who are homeless according to the fragility of their health and living situation VI Tool can be used to develop individualized housing plans

4 Colorado Counts: Communities working to house Coloradans in need Fifth state to join the 100K Homes Campaign Expanded vulnerability: Health vulnerability, AND Behaviors or circumstance that jeopardize their situation, At risk for system involvement, and Frequent User. 6 Mesa County Registry Week - March 5 to March 9, 2012 418 surveys completed- assessing homeless and at risk 75 families 49 youth (18-25 years) 92 veterans 151 chronic individuals/families

7 Pueblo County Registry Week Began June 30, 2012 80 Interviews (50 men, 27 women, 3 did not report) 34 medically vulnerable 18 Veterans 45 is average age 49 are chronically homeless 11 Families 6 are Youth/Under 25 years of age

8 Southwestern Colorado Registry Week July 16 to July 20, 2012 330 Interviews (181 men, 141 women, 7 did not report) 67 medically vulnerable 38 Veterans 77 Families 59 are Youth/Under 25 years of age
