5 Role of Referral Agency Who can Refer? Any health professional, community or social worker from a health or social agency within the Greater Wellington region.Any health professional, community or social worker from a health or social agency within the Greater Wellington region. 7 Referral Form Financial and Health Criteria Applicants MUST meet at least 1 criteria in both financial and health criteria

8 Financial Criteria Home Owner Occupant in the home currently has a Community Services Card or a Community Services Card endorsed SuperGold CardHome Owner Occupant in the home currently has a Community Services Card or a Community Services Card endorsed SuperGold Card Lead Tenant currently has a Community Services Card or a Community Services Card endorsed SuperGold CardLead Tenant currently has a Community Services Card or a Community Services Card endorsed SuperGold CardOR The Referral Agency confirms that whanau are experiencing financial hardship but do not qualify for a Community Services Card.The Referral Agency confirms that whanau are experiencing financial hardship but do not qualify for a Community Services Card. The client must meet one of the following examples to meet financial hardship: –Getting support from a budgeting service –Using food banks or receiving community food parcels –Accessing support from WINZ or other organisations Document what you see as financial hardship for when EECA undertakes an Audit

9 Health Criteria One or more of the occupants are under the age of 17 and/or over the age of 65;One or more of the occupants are under the age of 17 and/or over the age of 65;OR One or more of the occupants are at risk of developing, or have a pre-existing condition, affected by cold, damp housing e.gOne or more of the occupants are at risk of developing, or have a pre-existing condition, affected by cold, damp housing e.g –Respiratory condition (e.g. asthma, COPD, bronchiectasis, bronchiolitis) –Rheumatic fever –Coronary disease/ heart disease –Skin conditions (e.g. eczema, cellulitis) –Mental health (e.g. depression, anxiety etc) –Diabetes

13 Plans for 2014/15 For 2014/15, the Coalition is exploring an enhanced programme that may include the following components: –Curtains –Maintenance –Healthy & affordable heating –House assessments for efficiency measure –Health & social assessments –Householder education
