2 Funding and Staffing  Funding from the CDC  Budget: $274,000  FTEs: 1 FTE health program associate.5 FTE epi

3 Program Objectives  Increase the rate of: eye exams foot exams A1c testing Flu shots *Address health disparities *Wellness: pre-diabetes * Maintain a diabetes surveillance system

4 Current workplan  Conduct professional education  Institute worksite wellness at state agencies  Work with Hill CHC on pre-diabetes  Assist CHC in the dm collaborative  Work with partners to plan large scale diabetes awareness events…. continued

5 Workplan continued  Increase the number of NCQA recognized providers (by 10)  Promote the diabetes standards of care to PCPs  Plan, implement and evaluate a patient incentive program …. continued

6 Workplan continued  Promote flu shots through pharmacy and paycheck stuffers  Create and distribute a statewide diabetes newsletter
