1.Degree Plan- A listing of all of the courses required by the school to complete the Degree they are seeking. One thing to remember is that the document cannot be alterable. Meaning, when I click in that document in the soldier's file, I should not be able to alter the document. The most effective ways to accomplish this is to fill personalize your degree plan with your advisor, with pen-and-ink printing or annotations for your selected courses. Then have the advisor approve and sign. Scan this as a.pdf or image file and upload in GoArmyEd. 2.Cost Analysis/Statement- This is simply a line-by-line breakout of tuition and fees for the student to go to school at their chosen Home College. This too should be a scanned in document or screenshot, so it is not alterable. 3.Current Semester Class Schedule – listing of courses that the student is currently signed up to take. 4.Statement of Understanding (SOU)- This is the three page form that must be signed by the soldier and their Company Commander, or above. If other than the Commander signs line 14a of that document, the Authorized Signature Card must also be uploaded as a separate document to eFile. See attached blank SOU……. Degree Plan: All the Appalachian State University Major’s have degree plans posted at the following website: http://www.appstate.edu/academics/majors.php Each student applying for Federal Tuition Assistance Program (TAP) must print a copy of the degree plan for their academic Major. They must then personalize the degree plan to reflect the specific courses they are taking, and the advisor must approve and sign this document. Applying for Tuition Assistance Program (TAP) Personalized DEGREE PLAN Applying for Tuition Assistance Program (TAP) Personalized DEGREE PLAN Per AR621-5, chapter5, paragraph 5-4, b. a documented degree plan is generated by the institution specifically for the student that is requesting tuition assistance. The degree plan should be on an institution form, at best, or institution's letterhead. It should identify the student it pertains to. The degree plan should identify the student's courses, or category of courses, for the students program of study, i.e. Bachelor of Science in Criminal Justice. Ideally, the degree plan will list all historical courses (to include ACE evaluated credits and college credit exams that will be used toward the program of study), current courses, and projected courses. The degree plan should also be signed by a school official, usually the student's academic counselor.
