Step 1: select the correct requisition Requisitions available through standard register: 15-125040 general surgical pathology 15-121100A gyn 15-121100B placenta 15-125020 renal 0860 breast 0867 GI 0868 eye pathology dermatopathology

Patient’s full name: Last name, first name middle initial Complete date of birth or JHH medical record number (also called history number) Sex Clinic location (e.g. 109, WM205, 223, etc…) Procedure date and time (if applicable) specimen was collected *note collection time is required for breast specimens Attending /requesting physician: full name and NPI# Full name and address of other physicians who need a copy of the report Step 2: Complete Required Information

Clinical Information Clinical Description/ Prior History/ Working Diagnosis Number of specimens (containers) * Specimen description on the requisition must match exactly as labeled on the corresponding specimen container
