Quality Education for a Healthier Scotland How to re-submit Appraisal Forms for Appraisees (Primary and Secondary Care)

Why might I want to resubmit? You will have already submitted your appraisal forms, but require to add further documents, or amend appraisal forms – It could be at the Appraiser’s request; or – You submitted your forms without your MSF or PSQ reports (you were waiting for them to be collated/finalised); or – You amended your proposed PDP Resubmit will remove your previously submitted forms/documents, and replace with a refreshed version based on the current state of your draft appraisal forms (from the menu)

How to resubmit: 1.Amend as required on your draft forms from the menu (new documents, edit forms’ content, PDP etc) 2.Navigate to the interview details page (remember it’s the notepad icon you need to click) 3.Click “ Resubmit Appraisal Forms ” button at bottom of page

I can’t seem to resubmit? If Form 4 has been forwarded to you – you will not be able to amend & resubmit your forms, you will need to reject/return Form 4 to Appraiser first (via Change Request function) You will NOT be able to resubmit your forms if you are trying to amend & resubmit AFTER the appraisal was completed (i.e. Form 4 was signed off) – you can only add further documents to your appraisal at this point – Please see Powerpoint guidance on “Adding further documents after appraisal is completed”
