Transition plan Start at 16 Needs and outcome assessment – ascertain Fair Access to Care Eligibility/signpost to other agencies Carers assessment Reviews Linked to specific schools Work with Children Schools and Families and Youth Connexions

Prioritising cases for allocation of Social Worker in the Transition Team Mapping out complexity of needs Health needs – referral to Learning Disability Nursing in Transition Team Person Centred planning – focus on the young person’s aspirations, wishes and hopes for adult life.

16 - 18 Age 18 years Comprehensive assessment of needs Transition plan outlined, including social care and education Social worker allocated in Transition Team Learning Disabilities and Physical Disabilities Care and Support Planning commences Commissioning of services

18 - 25 Commissioned services reviewed and regulated based on changing needs At age 25 – referred to Community Learning Disability Team or Older People Physical Disability Team covering the area they live in Independent living, employment, day- activities
