Driver Safe Behavior Observation Card Driver: __________________________Date: _______________ Observer: _________________________________ 3 points of contact used when entering and exiting vehicle. Proper footwear with slip-resistant sole is worn. Ramp secured properly, easy access at top and flat/dry landing. Product is loaded/unloaded in 40 lb. maximum increments. Individual cases over 40 lbs. handled one at a time. Dolly/cart not loaded above chest level. Dolly/cart is pushed versus pulled whenever possible. If load must be pulled (up steps, etc.), weight and height of load is reduced to approximately waist level. Descends/ascends ramp while “in control”, not too fast, etc. Stretches periodically throughout day and hydrates/ drinks water often. Seat belt worn at all times. Cell phone never used when vehicle in motion. Instructions: Check boxes and provide positive feedback when driver demonstrates the behaviors consistently.
