3 REQUIRED LIST OF DOCUMENTS 1. Internet enrolment form. 2.Photocopy of passport. 3.Photocopy of a visa. 4.Insurance document. 5.Medical certificate confirming lack of contraindications to undertake studies. 6.One current photograph. 7.Secondary-school leaving certificate (the original or a official duplicate prepared by its issuer or by other entitled institution). 8.* Document confirming the knowledge of the English language (for foreigners applying for studies in English language).

5 PHOTOCOPY OF PASSPORT Photocopy of passport or other administrative identification document certified for conformity with the original by the SGH or a consul of the Republic of Poland (RP). To certify the photocopy of the document we need to see the original.

6 PHOTOCOPY OF A VISA Photocopy of a visa or a residency card or another document entitling its holder to remain within the territory of Poland, certified for conformity with the original by the SGH or a consul of RP. To certify the photocopy of the document we need to see the original.

7 VISA In Poland, alike other countries, regulations on entry conditions and visa requirements are complex and differ depending on your nationality and your current resident status therefore, please get acquainted with the specific information which applies to your case. 8 VISA According to the Polish law we have to ask for visa, but if you are unable to provide visa for the enrolment process, we will ask the Rector to issue a decision with condition to deliver visa at the later time. We will issue a decision on admission after we receive the set of your documents. This decision will enable you to apply to student visa. In the time being we can issue a statement saying that you have applied and are in the process of enrolment.

9 INSURANCE DOCUMENT a.Photocopy of the insurance policy in case of illness or a personal accident insurance policy for the period of education in Poland certified for conformity with the original by the SGH or a consul RP together with a certified translation into Polish, prepared by a certified translator entered in the list of translators by the Ministry od Justice of RP or certified by a consul of RP

10 INSURANCE DOCUMENT b.A European Health Insurance Card certified for conformity with the original by the SGH or a consul of the Republic of Poland. c.A declaration of a foreigner applying for admission to studies that he/ she will take out an insurance with the National Health Fund immediately upon commencing education.

11 INSURANCE DOCUMENT There are few options concerning insurance, you can either buy insurance or the other option is to declare in the registration system that you will take up insurance when you arrive in Poland. In the second option no translation and no certification of copies is necessary.

12 INSURANCE DOCUMENT Poland has the health care system based on general health insurance which entitles its bearer to free health services. An insured person and members of his/her family are entitled to free health services if they receive these services at health care providers that have signed contracts with the regional branch of the National Health Fund (NFZ), institution responsible for assuring health services and refund of prescribed medicine. The contract on providing health services specifies the kind and the scope of the services contracted by the given health provider with the National Health Fund.

14 MEDICAL CERTIFICATE CONFIRMING LACK OF CONTRAINDICATIONS TO UNDERTAKE STUDIES Medical certificate confirming lack of contraindications to undertake studies, in case the document is prepared in other language than Polish the certified translation into Polish is required. The translation must be prepared by a certified translator entered in the list of translators by the Ministry of Justice of RP or certified by a consul of RP. Medical certificate is a simple statement from the doctor saying that candidate is healthy and can take up studies. There is no special form.

16 A POLISH SECONDARY SCHOOL-LEAVING CERTIFICATE A Polish secondary school-leaving certificate (the original or a official duplicate prepared by its issuer or by other entitled institution)or secondary school-leaving certificate or other document obtained abroad, entitling them to apply for admission to studies at universities (the original or a certified copy by notary) or other document obtained abroad (the original or a duplicate prepared by its issuer or by other entitled institution), entitling them to apply for admission to studies at universities of all kinds in the country in whose system the institution granting the certificate operates, such certificate being considered equivalent to the Polish secondary school-leaving certificate pursuant to the regulations on nostrification of school certificates and secondary school-leaving certificates obtained abroad or considered equivalent to the Polish secondary school-leaving certificate or considered as entitling its holder to undertake such studies in the Republic of Poland under an international agreement may be admitted to first-degree studies (the legalisation requirement does not apply to the International Baccalaureat and European Baccalaureat diplomas)

18 A POLISH SECONDARY SCHOOL-LEAVING CERTIFICATE To receive legalisation of a document you are supposed to contact the Polish Embassy or Consulate in you country. 19 A POLISH SECONDARY SCHOOL-LEAVING CERTIFICATE b.Certified translation of the certificate into Polish, prepared by a certified translator entered in the list of translators by the Ministry of Justice of RP or certified by a consul of RP in office in the country where the document was issued You should translate all documents into Polish, we do not accept translations into English.

20 A POLISH SECONDARY SCHOOL-LEAVING CERTIFICATE c.Certificate confirming the equivalence of the foreign certificate with the Polish secondary school-leaving certificate, issued in conformity with the regulations on the nostrification of school certificates and secondary school-leaving certificates obtained abroad (the above certificate should be submitted within the deadline not exceeding the end of the semester)(does not apply to the International Baccalaurate and European Baccalaureate diplomas)

21 NOSTRIFICATION Nostrification is done in Poland after you arrive here. It is a procedure which only concerns secondary school-leaving certificate. In accordance with the Polish Law, each foreign secondary school-leaving certificate shall be submitted to a process of nostrification which is conducted by the Education Office in Warsaw (Kuratorium Oświaty w Warszawie), you have time till the end of the first semester to deliver the nostrification certificate. http://www.en.kuratorium.waw.pl/en/site/285/nostrification-of-the- certificate.html http://www.en.kuratorium.waw.pl/en/site/285/nostrification-of-the- certificate.html Please take into consideration the fact that the whole process takes around 1 month.

22 NOSTRIFICATION For nostrification of your secondary school-leaving certificate you need: 1. original of secondary school-leaving certificate + certified translation into Polish + apostille or legalization 2. Statement issued by the Polish Consulate in the country of certificate’s origin or the Embassy of the country of certificate’s origin in Poland, which reads: "I declare that Mr/Mrs...... is eligible to undertake studies in ………… (the country of certificate’s origin) based on certificate no....... issued on............. „

23 NOSTRIFICATION For nostrification of your secondary school-leaving certificate you need: 3. photocopy of the identification document 4. application form from http://www.kuratorium.waw.pl/files/f-144-2-wniosek.pdf

24 Foreigners applying for admission to studies conducted in the English language submit a document confirming the knowledge of the English language (one of the listed below) DOCUMENT CONFIRMING THE KNOWLEDGE OF THE ENGLISH LANGUAGE (one of the listed below) a.Certificate in Advanced English (CAE), Certificate of Proficiency in English (CPE), Certificate in English for International Business and Trade (CEIB). b.Test of English as a Foreign Language (TOEFL)- at least 85 points in the Internet- Based Test (iBT) version or at least 180 points in the Computer-Based Test (CBT) version. c.Spoken English Test (SET) for Business: Stage C „Mastery” level. d.London Chamber of Commerce and Industry (LCCI): English for Business Level 3, English for Business Level 4.

25 e.International English Language Testing System IELTS- at least 6 points. f.City & Guilds Level I Certificate in ESOL International (reading, writing and listening) Communicator (B2) 500/1765/2; City & Guilds Level 2 Certificate in ESOL International (reading, writing and listening) Expert (C1) 500/1766/14; City & Guilds Level 3 Certificate in ESOL International (reading, writing and listening) Mastery (C2) 500/1767/6. g.Certificate, diploma or another document confirming graduation from institution at least on secondary level where classes were conducted in English. The document confirming the knowledge of English may be submitted in the form of a photocopy of the document, certified for conformity with the original by the SGH or a consul of the Republic of Poland.

26 You can submit the documents by post or personally to Admission Center at SGH, we require the documents to be in original paper versions. There is no possibility to upload documents or send via internet. For first verification you can send the scans of your documents via e-mail and we will review them and let you know if there is something else we require. There are two rounds of submission of documents. If you do not manage to prepare your documents and send them till 18 July, you can submit the documents in second round 26.08-05.09. You can submit the documents in either round, first-come-first-serve rule does not apply here.

27 CERTIFIED TRANSLATIONS You should translate all documents into Polish, we do not accept translations into English. We do not accept translations done by notary. We require certified translations into Polish of required documents. There are two ways to translate the documents. Translation can be done by: -certified translators entered on the list of Ministry of Justice of Poland or -normal translator and then certified by the Polish embassy/consulate in your country. It is a normal procedure and it done at the embassy/consulate.
