2. 2 Form 471 Receipt Acknowledgment Letter Confirms receipt of the Form 471 Verifies contract information Confirms successful data entry of application into the SLD system (check for errors by SLD or applicant) Includes information on what can and cannot be corrected at this point

3. 3 Form 471 RAL The RAL is received after data has been entered into the SLD computer system (either online by the applicant or by the SLD staff) The RAL will list some of the information from the submitted 471 Some corrections can be made on this letter Corrections must be done immediately upon receipt of the RAL – 3 week deadline for most corrections

4. 4 Form 471 RAL The RAL can be used to correct data, like: –Contact information –SPIN data entry errors –Duplicate or missing line items Can reduce or cancel a funding request

5. 5 Form 471 RAL-Corrections Photocopy your RAL Draw a line through each incorrect item, and mark clearly next to it what the correct information should be Send to the SLD within three weeks Use proof of receipt delivery system or fax and keep fax confirmation page Keep a copy of corrections for your records
