WHAT IS COMMUNITY HEALTH CARE? Community Health Care: care provided to people who live within a specific geographical area or have common needs. Designed to meet the needs of the patient as he/she moves among healthcare settings with in the healthcare system. Community Health Nursing: focus is on the individual and family healthcare needs.
