MIP members pay the invoice upfront and send to GMC :
The detailed original invoices showing the dates of service and the name of the patient, OR
GMC MIP – Refund Claim Form dully filled in, stamped by the Attending Doctor and signed by both.

8. How to submit a claim? Inpatient hospital care :
If MIP Members pay the hospital fees directly to the provider, the following documents should be submitted for reimbursement :
The original invoice,
The discharge medical report,
A receipt of payment.
