3 Today we will cover An introduction to Self-Directed Support Transformation programme – update Video and Question & Answer session YOUR views – how you can shape things!

4 National context All started with “Putting People First” (2007) and a series of national pilots DH released “Transformation Grant” to make this happen by 2011 All Local Authorities in the UK are introducing Self-Directed Support (SDS)

7 Definition of Self Directed Support Simply put, SDS is a new model… where service users & carers who are eligible for social care … are offered a personal budget to spend on meeting their needs!

8 But Self-Directed Support is more than this:

9 Self-Directed Support Supported Assessment Questionnaire Co-production with the customer Support Plan Person- centred Professional assessmentStreamlined Resource allocation Based on need Brokerage By in-house team or 3 rd party Review 6 weeks and a year The new system The social worker role is key

10 Assessment The professional assessment is still crucial The SAQ is short – and the customer should take the lead In future, resources should be allocated more equitably – based on need

11 Support Planning A person-centred approach Creativity and flexibility are important; as are… Risk assessment and contingency planning This is a conditional resource entitlement - activities must meet the required outcomes Support plans are signed off by the council

12 Brokerage Brokers make the support plan a reality They are specialists, and provide an independent view Brokers present options to the customer In Hillingdon we have an in-house team

13 The role of the council First and foremost - a responsibility to vulnerable people Signing off support plans To create a market infrastructure To fill gaps in the market To accredit and quality monitor providers

14 Our approach in Hillingdon – when is this happening? A number of pilots are currently taking place From 1 st August 2010, existing customers who receive day or domiciliary care will receive Self- Directed Support at their next review From 1 st April 2011, all new customers will receive Self-Directed Support

15 Our approach in Hillingdon – aims Providing full support for customers to make the most of the system and find the right support options Giving real choice in the range of support options available and control in how they are received Helping people maintain independence and enjoy fuller lives Extended information and advice support for all

16 Self-Directed Support aims to bring the following benefits to Hillingdon’s staff: Greater freedom to find the right solutions Room for creativity and innovation Less bureaucratic processes Less paperwork Improvements to Protocol Better outcomes for residents

17 Self-Directed Support aims to bring the following benefits to Hillingdon’s residents: Greater choice in support Greater choice in services to meet need Putting customers in control Boosted day-to-day independence levels Increased autonomy in decision making Greater equity Better outcomes
