5. Kasturba Gandhi Balika Vidyalay Janpriya Nagar, Janpriya Vidyalay (H.S.) Field Visit Report – Team - 1 General Operation :  Teaching Learning Materials not available or displayed;  Enrollment Register not properly maintained  Health Card of the girls enrolled were not properly maintained. Rather, on a printed card, name and age of the girls were mentioned without any findings of the health check up.  Practice of Regular Health Check up not followed;  No photographs of the girls or the identity card were available ; 19 th Review Meeting of SSA - Kolkata

8. Field Visit Report – Team - 1 Operation of Bank Account,  No separate Bank Account of KGBV  Amount transferred from DPO to School from where amount is withdrawn and spent in for KGBV;  No Cash Book is maintained to record the cash transaction at KGBV;  Huge amount is withdrawn and spent. On 25th September’2008 Rs. 2.50 lakh was withdrawn in cash;  No account of expenditure incurred is recorded before next withdrawal;  No Cheque Issue Register is maintained 19 th Review Meeting of SSA - Kolkata

Field Visit Report – Team - 1  Stock Register in respect of consumable items or the fixed assets acquired is not maintained;  No physical Verification of Stock has been conducted either by the Centre / DPO or the Auditors  No visitors register is maintained;  No schedule of timing for the appointment of girls with their parents is scheduled or displayed ;  Payment vouchers are nor passed and authorised for payment  Though Accountant is appointed but without any work; 19 th Review Meeting of SSA - Kolkata
