5 Eligibility Active State employees, their lawful spouse or registered domestic partner and unmarried dependent children. Children, natural, adopted, or step children under age 23, or any age if incapable of self-sustaining employment due to mental or physical disability. Dependent children in military are not eligible.

6 Employment Category and Benefit Level 1 (per plan year) Bargaining Unit 5 & 7 employees 7 sessions per problem type for employees Exempt, managerial, supervisory & confidential employees of the California Highway Patrol 7 sessions spouse or Reg. Domestic partner Employees, managers, supervisors & confidential employees in B.U. 6, 7 & 8 including seasonal and intermittent firefighters. 7 sessions total for dependent children * Sessions per problem type

7 Employment Category and Benefit Level 2 (per plan year) All California Highway Patrol, Department of Forestry and Fire Protection (State Fire Marshall), and Department of Corrections and Rehabilitation employees (unless listed in Level 1) 3 sessions per problem type total, shared between employee, spouse or registered domestic partner and children

8 Employment Category and Benefit Level 3 (per plan year) All other employees 3 sessions total for employee 3 sessions total for spouse or registered domestic partner and children

9 Problem Types Substance abuse Alcohol abuse Marital & family issues Emotional, personal & stress concerns

10 Confidentiality Strictly maintained as required by law Legal exceptions » child or elder abuse » intent to commit homicide or suicide » court order

11 Web-Video Consultation How Web-Video Consultation Works: Make an appointment as you usually do, call EAP at (866) 327-4762. You’ll be asked if you want a web-video consultation, and if you do then we’ll let you know exactly what you need to connect. If you have an internet connection and a web camera you’re probably set to go!

12 Work & Life Services Legal Financial Child & Elder care Identity Theft Recovery Services Daily Living These services do not count against your clinical sessions.

13 Legal Consultations Up to 30 minutes per session, face-to-face or telephonic. 25% off hourly rate if you retain their services. Family issues, Criminal issues Estate planning/wills/trusts Real Estate, Landlord/tenant Small claims, Personal injury/malpractice Bankruptcy

14 Financial Consultations This service is designed to address budget and credit issues, and you are entitled to as many telephonic financial consultations as you need per separate financial issue. Bankruptcy, Credit Card debt Budgeting First time home buying Foreclosure prevention College fund planning

15 Elder Care Up to 60 minutes per telephonic consultation session. Identify community resources Evaluate financial & emotional stresses Review techniques to monitor/evaluate

16 Child Care Up to 30 minutes per telephonic consultation session. Assess your needs Identify community resources Learn what to look for

17 Identity Theft Recovery 30 minutes per telephonic consultation 30 minute consultation with certified consumer credit counselor Objectively assess your situation, create an action plan and provide the knowledge and tools to implement the plan most effectively Counselor will help assess your risk level and identify steps to resolve potential identify theft

18 Daily Living Services Referrals to Help with everyday errands, travel, event planning and more. Locate 3-5 businesses/vendors nationwide such as auto repair, home maintenance, travel agency, sales, landscaping, etc. 24-hour request access Research special requests Response within 72 hours
