O.S.C.V.I. Attendance Flowchart Student is Absent Authorized by phone call before the end of the school day. Information on the class attendance sheets. Authorized by phone call after attendance sheets are created. Admit slips are given to the home form teacher to be distributed. Authorized by a note. The student takes their note to the office. The office keeps the note and creates an admit slip for the student. Unauthorized student. No admit slip and no reason. The student must go to the office immediately. Office will phone home or work number. 1

Unauthorized Absence 1 Office phones home or parents’ work number. The student is authorized and is given an admit slip before going back to class. The student is still unauthorized and receives at least one detention. The student receives an admit slip and is returned to class. If the unauthorized student brings in a note before school starts the next day The detentions are erased and a new authorized admit slip is given out.

Attendance System Teachers Duties Within the first five minutes of class the attendance must be filled out carefully and placed in the attendance folder in the hallway. The DL class will collect the attendance sheets and deliver them to the office. Teachers will be called if the attendance has not been received. All the attendance sheets are scanned into Trillium. The attendance sheets are placed back into each teacher’s folder. If a student was absent and is not on the attendance sheet or does not have an Admit slip then the student is sent down to the office. 1

Lates The student is late for class. The student arrives for class before the attendance sheet is picked up. The teacher bubbles in the late (L) bubble. If the student is late after the attendance sheet is picked up then the student must ask for a filled out late slip (at the teacher’s convenience). The student will deliver the late slip to the office on the same day.
