Ethics Application Info (see Honours Thesis Handbook) Departmental Ethics Review Committee (DERC) Michelle Manios regadmin@uwaterloo.caregadmin@uwaterloo.ca Step 1:TCPS2 (CORE) Ethics Tutorial (must be done *before* ethics application) http://pre.ethics.gc.ca/eng/education/tutorial-didacticiel/ Step 2:Ethics Application (must be done *before* data collection) Form 101 Online: Create and submit online http://iris.uwaterloo.ca/ethics/form101 Supporting Materials: Recruitment Materials REG (Sona Description), PSYCpool (email/phone scripts), SLC (Flyer) Information/Consent Letter (+ Post-debriefing Consent if deception) Feedback Letter (+ Oral Debriefing if deception) Survey/Questionnaire/Interview items/Stimulus Appendices Research Proposal (*if available but not required) Print and sign ( student + supervisor ) 1 (ONE) copy double-sided Submit to DERC mailbox (3 rd floor PAS mailroom) – not ORE office Exemption from ethics review, see next page Step 3:Research Proposal reviewed/approved by supervisor Step 4:Oral Presentation (to receive departmental approval of project) Step 5: Upon receipt of ORE Full Clearance, data collection may begin Step 6: For later revisions, submit Form 104 Modification, revised materials Warning!! 3 – 4 weeks required for Full Ethics Clearance

Exemption from ethics review Not sure if you need to submit an ethics application? If the project involves only supplemental or re-analysis of existing data *and* There will be no new data collected *and* the existing data was collected under an active, approved ORE Number a) Ethics application (Form 101) NOT REQUIRED IF you were identified as a ‘Student Investigator’ under that ORE Number b) Ethics modification (Form 104) REQUIRED IF you were NOT identified as a ‘Student Investigator’ under that ORE Number the original ORE number DID NOT mention the honours thesis In either of case a) or b) you must notify the DERC officer prior to starting the project who will process exemptionDERC officer Submit Form 104 (if necessary) await approval of the ethics exemption from the 499 Faculty Advisor complete the TCPS2 CORE Tutorial In all other scenarios a Form 101 application is required. If you are unsure, contact the DERC OfficerDERC Officer

Discuss with your supervisor i.e., need to recruit specific groups Advise/discuss with REG Coordinator early on Don’t wait until the term when you are planning to collect data as these surveys are created in advance of each term
