4. Custom build (or use one of the sample) questionnaires. Select a rating scale that meets your requirements. Add free text boxes for written comments. Slide 4

5. Customise the layout of the report. Add additional pages such as development planners, and tips on giving feedback. Change the ordering of the pages. Slide 5

6. View a sample of the finished report. Continue to edit the layout and the questionnaire. Slide 6

7. Distribute the questionnaire to all the people involved. Slide 7

8. Allow time for the people to complete the questionnaires. Slide 8
