Open sky with increased risk of positive pressure, scleral collapse Forward herniation of the iris / posterior capsular rupture and vitreous loss Increased risk of choroidal hemorrhage Uncertainty of exact placement of the posterior chamber IOL

10 SURGICAL ADVANCES Suturing of donor after nucleus expression with interrupted sutures I & A thru graft host junction Implantation of posterior chamber IOL thru graft host junction

11 Results 32/34 eyes had successful implantation of the PC IOL
One eye had one haptic in front of iris, successfully repositioned No incidence of capsular rupture, vitreous loss or lens dislocation All grafts were clear

12 Conclusions This new technique allows successful placement of a PC IOL in spite of positive vitreous pressure during the triple procedure. All grafts were clear; with no incidence of capsule rupture, vitreous loss or lens dislocation.
