13. Completing the ASAP Under Other Role/Report Type, Choose WIC LAP

14. Completing the ASAP Enter Today’s Date using MM/DD/YYYY format

15. Completing the ASAP Use only if you complete the LAP at more than one agency

16. Completing the ASAP Click the “I Agree” Button

17. Completing the ASAP Click the “Submit Form” button at the bottom of the page
