3 Basic Steps  Secure the scene  Control hazardous conditions  Provide care  Call EMS and police if necessary  Isolate the scene  Notify OSHA, TRC, TxGLO, Insurance carrier  Gather evidence  Identify and interview witnesses

5 Accident Scene Preservation Secure the area Photograph the area from various angles –Use an item of known length to give perspective to objects Draw a picture or diagram the scene –Details are critical Collect samples, if applicable

6 Accident Scene Preservation Collect written documents –JSAs, log books, work permits, contracts, training records List eye witnesses and contact information (interview ASAP) Interview emergency personnel about the conditions upon arrival List all equipment and materials involved

7 Accident Scene Preservation List environmental factors –Wind, temperature, light, dampness, time Determine the original location of all items Obtain copies of police report and any photographs taken

8 The Four P’s of Evidence Physical evidence –Tools, equipment, spills, leaks, debris Paper evidence –JSAs, procedures, training records People evidence –Written statements ASAP Picture evidence –Photos and diagrams
