1 Mynydd Isa Branch Closure Roseneath Medical Practice is due to move into new custom built premises in early 2015. The Practice wishes to ensure that all patients are given the best possible service.

2 Mynydd Isa Branch Closure In order to do this as effectively as possible the Practice wishes to move away from old out of date premises that are not fit for purpose. As a consequence the Practice has formally applied to Betsi Cadwaladr University Health Board to close the branch surgery at Mynydd Isa.

3 Mynydd Isa Branch Closure The utilisation of the branch surgery at Mynydd Isa was never considered a long term solution and was only needed due to the lack of room space in the existing Health Centre in Buckley.

4 Mynydd Isa Branch Closure The new Buckley Medical Centre was expected to be available much sooner than has actually happened. The project has been beset by delay after delay.

5 Mynydd Isa Branch Closure The new Buckley Medical Centre will provide more than adequate room space for the entire Practice team.

6 Mynydd Isa Branch Closure The reason for the proposal is that there are longstanding problems associated with the operation of the branch surgery.

7 Mynydd Isa Branch Closure Schedule 1 of the National Health Service (General Medical Services – Premises Costs) (Wales) Directions 2004 sets out the minimum standards for Practice premises. Both of the current surgery premises do not comply with the minimum standards required.

8 Mynydd Isa Branch Closure The branch surgery is not equipped to the same level as the main surgery. There is no ECG machine at the branch surgery so if a patient is seen there and requires an ECG they have to go to the main surgery.

9 Mynydd Isa Branch Closure There is also no nebuliser facility at the branch surgery. The mains powered ophthalmoscope/auroscope used at the main surgery are far more effective than the battery powered ones used at the branch surgery.

10 Mynydd Isa Branch Closure There is only one telephone line at the branch surgery and a clinician attempting to make calls can often find the line in use by patients or the receptionist. This can also be frustrating for patients trying to get through when the line is in use.

11 Mynydd Isa Branch Closure Stock levels at the branch surgery are lower than at the main surgery with running out a frequent occurrence.

12 Mynydd Isa Branch Closure The computers at the branch surgery are less reliable, problems often occurring due to roaming profiles and setup issues. Printers frequently cause problems due to profiles and setup.

13 Mynydd Isa Branch Closure There is no onsite IT support so problems take longer to resolve with IT staff having to travel from the main surgery to the branch surgery to address any problems.

14 Mynydd Isa Branch Closure IT problems can result in clinicians having to hand write patient records and prescriptions which have to be entered into the computer record later increasing the workload.

15 Mynydd Isa Branch Closure It is often difficult for the IT helpdesk to resolve IT problems due to having only one phone line. There is no mobile signal at the branch surgery.

16 Mynydd Isa Branch Closure Documentation, referral letters, prescriptions may be left at the branch surgery when they should be brought to the main surgery.

17 Mynydd Isa Branch Closure If a document has not been scanned or notes have not been summarised there is no access to this information at the branch surgery. This is especially relevant for new patients who have little data recorded.

18 Mynydd Isa Branch Closure Patients and clinicians frequently go to the wrong surgery. Clinician’s time is lost travelling too and from the branch surgery.

19 Mynydd Isa Branch Closure There is effective clinical peer support at the Buckley surgery but clinicians are often on their own at the branch surgery.

20 Mynydd Isa Branch Closure There is only one receptionist at the branch surgery so a lone worker policy had to be put in place.

21 Mynydd Isa Branch Closure The appointment book is complicated by having the branch surgery. It is much more difficult to cope with unexpected absence due to sickness at the branch surgery.

22 Mynydd Isa Branch Closure The above longstanding problems associated with the branch surgery impact directly on service to our patients. Patients can be inconvenienced by the lack of equipment and by IT problems.

23 Mynydd Isa Branch Closure The full range of patient services is not available at the branch surgery which is to the disadvantage of the patients attending there.

24 Mynydd Isa Branch Closure Consolidating the Practice team on one site following the move to the new Buckley Medical Centre will result in significant benefits.

25 Mynydd Isa Branch Closure Full range of services available to all patients attending the surgery Fully Disability Discrimination Act compliant premises to meet all patients needs More efficient use of clinicians time being in one location

26 Mynydd Isa Branch Closure Effective clinical peer support, this is where the doctors support each other and discuss cases. More effective equipment and stock management Full access to all patient information
