Administrative Compliance Tender document returned in original form Tender document must be completed in full and in black ink, all items completed as instructed and Summary Page has been signed. Tenderer must sign and complete the Form of Offer AND the following SBD Forms SBD1, SBD 2,SBD4, SBD 6.1, SBD8, SBD9 Company must Submit certified proof of registration with South African Registrar of Companies (CiPC). Company must Submit proof of Ownership and Shareholding with relevant certified ID documentation Company must Submit a detailed Company Profile including CV’s and Certified Qualifications indicating Registration numbers from CIDB,NHBRC,ESCA, CESA and\or SABTACO. Company must Submit a valid Original Tax Clearance Certificate. (No colour scanned or certified copies are allowed.)

Administrative Compliance ( Continues) Company must Submit Certified B-BBEE Certificate as per Legislation and or provided a certified In-Process Letter from Accredited Auditor. Ensure that the BBBEE verifier is accredited with SANAS. Website: www.sanas.gov.zawww.sanas.gov.za Company must Submit a valid Letter of Good Standing from Compensation Commissioner Company must Submit a Municipal Account not older than 3 months Company must Submit a proof of valid and current Professional Indemnity Insurance (Reference Table 1 on Page 6 for minimum PI Insurance required) from Accredited Financial Service Provider Your company will be disqualifies if it is involved in more than one Joint Ventures and\or its member or employees are registered in more than one Joint Ventures.

Administrative Compliance (Continues) Company must Submit a certified proof of individual members Labour Intensive NQF5 & NQF7 achievement certificate. All Engineers and companies(y) must be registered with either one of these association: ECSA CESA SABTACO Company must Submit a certified financial statement to reflect annual turnover Tender completed Bank Details and provided a cancelled cheque / Bank Letter

Administrative Compliance (Continues) Joint venture: Attach a JV agreement which includes percentage contribution to the project. All parties involve in the JV must submit valid original TCC. Joint Venture agreement and percentage involvement for each company All Joint Venture Agreements must be notary.

Stage 2: Functional or Technical Compliance Functional Compliance Functionality will be out of 90 percent. oFor categories 1- 4 the minimum threshold is 60%. oFor Categories 5 – 6 the minimum threshold is 70%. Bidders who attain the minimum or more will move over to the next stage: Stage 3

Stage 3: Price and Preference Price and Preference ( BBBEE ) o90 is for Price All bidders will be awarded 90 points as intuition fees scales are applicable. o10 is for Preference\ BBBEE

CLOSING AND OPENING OF BIDS Bids received before the closing date and on the closing date before 11:00 am will be accepted. The closing date is 05 February 2015 for bid no: W 1014 WTE and 12 February 2015 for bid no: WP 0485 WTE Tender box at Zwamadaka Building.157 Schoeman Street, Open 24/7 including Public Holidays. No late bids will be accepted. We check time using Telkom line 1023.
