Webinar will be hosted on October 2 nd at 10:00 (it will be recorded) Do not start interviews until mid-October Read over guidance carefully Ask for help in completing interviews and scoring Submit the “Tri-Ethnic Readiness Assessment Results” form Keep documents on file for one year

Complete during a coalition meeting Host a discussion about the process during the meeting Submit “Reasons for GAP Score” document to IDPH Details on averaging the scores can be found in the Guide Include a summary of the discussion for each section and the averaged scores for Years 1 and 3
