Review of the New DHMH 896 Form Revised February 2014 Changes:  The “LOST OR DESTROYED RECORDS” section was removed  Under “MEDICAL CONTRAINDICATION”, Health care providers must document the “reason for the contraindication”.  “Immunization Requirements” on the reverse side of the form have been updated to include the new requirements for Tdap and MCV (meningococcal vaccine). 9
